A clinical Study of Postoperative Prognosis Associated with Surgical Treatment of Peptic Ulcer Disease in Cirrhotic Patients

Abstract
Liver cirrhosis is frequently associated with peptic dulcer disease in Korea. Surgery in liver cirrhoties has a high mortality and morbidity, especially when abdominal operations are performed.
The authors have investigated numerous factors that may influence the postoperative prognosis following peptic ulcer surgery in 37 patients with liver cirrhosis admitted to the Pohang St. Mary's Hospital during the period between July, 1984 and
July,
1994.
To determine possible prognostic factors, various clinical and laboratory findings were examined for their relationship to postoperative mortality using Chi-square test.
@ES The results were as follows:
@EN 1) Gastric ulcers were more frequent than duodenal ulcers in liver cirrhotics.
2. Respiratory failure was the most frequent postoperative complication.
3. Postoperative mortality was higher in emergency operation than in elective operations.
4. Statistically significant prognostic factors affecting mortality rte in this study were the following increased PT by more than 2 sec. ascites, receiving blood transfusion over 10 units and urine output output under 30 ml/hr(P<0.05).
In conclusion, proper evaluation of the above risk factors, optimizing liver and renal function preoperatively and avoiding excessive blood loss is important in reducing the postoperative mortality of peptic ulcer surgery in patients with liver
cirrhosis.
